Insurance Forms

Filling of insurance forms is a service not covered or paid for by Alberta Health. The most common forms that your surgeon will fill are related to short or long term disability following surgery or another procedure, or an attending physician’s statement. These forms reflect the expertise, long training, and expert professional opinion of your physician.

Payment is required prior to forms being released, and these forms will form part of your medical record.

There is a one time fee for forms, payable by Interac e-transfer.

• Simple forms (i.e. 1-2 pages, limited information) - $100

• Complex forms (longer forms, requiring the physician to review the chart) - $150
